Scan your certificates so every IFSAC or ProBoard seal and number is legible. The FAQ warns that if the seals and numbers cannot be read, your application will not be processed.
Two published substitutions help some applicants. If you are waiting on your seals, include your transcript and a letter from the certifying body stating the seals have been applied for. A current EMR, EMT, EMT-A, PCP, or ACP licence plus a current CPR Level C with AED certificate can replace Standard First Aid, and an out-of-province driver's licence needs documents proving it is equivalent.

Medicine Hat uses the University of Alberta's Faculty of Kinesiology, Sport and Recreation for its physical test, and the cost is your responsibility. The department notifies candidates of available testing dates, but you book and pay for the test yourself.
The U of A lists the test at $399 plus GST, payable in full at booking, with a non-refundable $150 portion, and testing takes about three to four hours at the Universiade Pavilion (Butterdome) in Edmonton. A physician-signed medical clearance form is mandatory, so book a doctor's appointment as soon as you register.
